Can I tile right over the existing splash?

Occasionally, if the existing surface is flat, sound and the added depth will not foul the counter or trim. Usually removing is cleaner and not much more work. See removal & replacement.

Do I need waterproofing?

For a vanity splashback, surface prep and correct caulking are usually enough. Where tile sits in a genuinely wet zone — inside a shower surround or around a tub deck — proper waterproofing is not optional, and we will tell you if the job crosses that line.
